Chapter Sheet

I am in the process of opening up a video slideshow business. I have been reading some of the topics on here and a lot of people are talking about a chapter sheet that they have their clients fill out. I was wondering if anyone can e-mail me a copy of a samle chapter sheet? I am wondering what all is on this sheet and the best way to take the orders of my clients.
